Terms of Use

These Terms constitute a legally binding agreement between you and Mino Medical, Inc. regarding your participation in the PA-SBA Member Health Benefit programme. They apply to all interactions with the Programme, including the website, enrolment process, physician intake, and ongoing member communications.

We reserve the right to modify these Terms at any time. Material changes will be communicated to enrolled members via email. Continued participation in the Programme following notice of changes constitutes your acceptance of the revised Terms.

These Terms should be read alongside our Privacy Policy, which governs the collection and use of your personal and health information.

The Programme is available exclusively to active members of the Pennsylvania State Troopers Association (PA-SBA) and their eligible dependants, as defined by the applicable collective bargaining agreement or benefit plan documents.

You must be at least 18 years of age to enrol. Enrolment of eligible dependants under 18 must be completed by a parent or legal guardian who agrees to these Terms on their behalf.

Eligibility is subject to verification. Mino and PA-SBA reserve the right to confirm your membership status at any time and to suspend or terminate access if eligibility cannot be confirmed.

The Programme is not available to individuals who are not current PA-SBA members in good standing. Eligibility ceases upon termination of PA-SBA membership unless otherwise specified in the benefit plan documents.

You agree to provide accurate, complete, and current information during enrolment and throughout your participation in the Programme. Providing false or misleading information may result in termination of your access.

You are responsible for attending scheduled physician intake appointments and following up with the Mino medical team as directed. Failure to engage with the Programme does not entitle you to a refund or alternative benefit.

You acknowledge that the Programme provides physician-supervised health and wellness services and is not a substitute for emergency medical care. In the event of a medical emergency, you should contact emergency services immediately.

You agree not to misuse the Programme, including attempting to obtain services for ineligible individuals, sharing access credentials, or using the Programme for any unlawful purpose.

The Programme and all related services are provided on an "as is" and "as available" basis. To the fullest extent permitted by applicable law, Mino Medical, Inc. and PA-SBA disclaim all warranties, express or implied, including warranties of merchantability, fitness for a particular purpose, and non-infringement.

Neither Mino Medical, Inc. nor PA-SBA shall be liable for any indirect, incidental, special, consequential, or punitive damages arising out of or related to your participation in the Programme, even if advised of the possibility of such damages.

Our total liability to you for any claim arising out of or relating to the Programme shall not exceed the total amount paid by you (if any) for the Programme in the twelve months preceding the claim.

Nothing in these Terms limits liability for death or personal injury caused by negligence, fraud, or any other liability that cannot be excluded or limited under applicable law.

All content on this website and within the Programme, including text, graphics, logos, images, and software, is the property of Mino Medical, Inc. or its licensors and is protected by applicable intellectual property laws.

You are granted a limited, non-exclusive, non-transferable licence to access and use the Programme solely for your personal, non-commercial use as an eligible PA-SBA member.

You may not reproduce, distribute, modify, create derivative works of, publicly display, or otherwise exploit any content from the Programme without the prior written consent of Mino Medical, Inc.

The PA-SBA name, badge, and associated marks are the property of the Pennsylvania State Troopers Association and are used with permission. Nothing in these Terms grants you any rights in those marks.

In the event of a dispute arising out of or relating to these Terms or the Programme, the parties agree to first attempt to resolve the matter informally by contacting Mino at [email protected]. We will endeavour to respond within 10 business days.

If informal resolution is unsuccessful, any dispute shall be resolved by binding arbitration administered under the rules of the American Arbitration Association (AAA), except that either party may seek injunctive or other equitable relief in a court of competent jurisdiction.

These Terms are governed by the laws of the Commonwealth of Pennsylvania, without regard to its conflict of law provisions. Any arbitration or court proceedings shall take place in Pennsylvania.

You agree that any dispute resolution proceedings will be conducted on an individual basis and not as part of a class, consolidated, or representative action.
